Both.. which is it? Was he formerly chairman? Or was the Teachers Association disbanded?

If the person spoken about was the Vice Chairman of Teachers Association than Former Vice Chairman of Teachers Association is correct. If the person spoken about was the Vice Chairman of a Teachers Association that no longer exists than Vice Chairman of former Teachers Association is correct.
