1) Mary: "I know a girl from London". John: "Why (do you know her)?";
2) Mary: "I know a girl from London". John: "How (do you know her)?" ?

Why: ask for what reason or purpose.
How: ask about the condition or quality of something, in what way or manner.
I think you mean the second, so the right is:
"How (do you know her)?" ?

The question is 'How do you know her?'.
We ask 'how', because you are really asking how you met, or what the connection is.
