Simple Past or Present perfect?
Exercise my Solutions ( I am not sure if I am wrong, can someone explain why the other solution is correct?)
1. Yes, of course, I made lunch and wrote a short story for English.
- completed action? simple past
2. Have you helped your Grandma yet?
- signal word yet present perfect
3. Yes, I have. I have been in town...
- I am still in town - present perfect
...and I took out the rubbish.
- Completed action?
4. Have you taken Sam for a walk?
- idk
5. Yes, I have, and I fed him too.

To be honest some of these could be either present perfect or past. All your answers sound ok. The last one could be “have fed”.
Number 3 is odd. It suggests Grandma is in town but it doesn’t really seem relevant.

3. I was in town and I took out the rubbish.
(In 4 and 5 Lesli's response "Yes, I have," suggests her mother used "Have you taken..", "Have you helped.." etc. but I think more likely a native speaker would use the simple past ie "Did you take Sam for a walk?" "Yes, I did, and I fed him too" for completed actions in the past.)
