"For" is better. Actually you would probably say "leave me a note".

I think they both correct because if u talking about yourself is you can leave the note for me and when the notes belong to someone else then you can say leave the note to me so I can give to him/or her

In general, use "for" if you are to receive the note. The note is left for you. The only way "to" can work is if you are to write the note: "You make the phone call, and leave the note to me" (ie. leave the writing of the note up to me; that'll be my job).

They mean 2 different things. "leave the note for me" = leave a note for me about something "leave the note to me"= Don't write the note, leave the writing of the note to me.
