Is this correct "Anong nangyari mo?" and also "What is happening?" Is this correct "Ano ba ang nangyari?" Please give me the correct translations, thanks :)

No, the correct translation for "What happened to you?" is "Anong nangyari sa'yo?". "Ano ba ang nangyari?" is "What happened?". "What is happening?" is "Ano ba ang nangyayari?" Did you see the difference? :)

Anong nangyari? What happened? Anong nangyari sa iyo? What happened to you? Ano ba ang nangyari? What happened? (but in a manner where you are trying to know the cause of a problem) Anong nangyayari? What is happening? You repeat the first syllable of the verb (pandiwa) to say its form in present tense.

"What happened to you?" -> "An'yare sa'yo?" (slang) "What happened to you?" -> "Anong nangyari sa iyo?" (slang)

"What happened to you" in Tagalog you can say, "Ano ang nangyari sa iyo?" Sometimes we use the single quotation mark (') to make sa iyo into sa'yo. (sounds better and slang ^^ ) Like in English, example: 1. I am about to do that/ I am 'bout to do that. 2. I like you because you are nice/ I like you 'cause you are nice. It's really important to know the present tense, past tense and the future tense of the words of the language that you want to learn. "What is happening?" is present which means it's happening, of course ^^ So in Tagalog you can say "Ano ang nangyayari?" Present Tense: Nangyayari Past Tense: Nangyari Future Tense: Mangyayari Sometimes, using the word "ba" isn't really important. I don't know if there's an English word for that but for me, I'm only using it if I am desperate or really confuse in what I am asking. I hope it helped you. :)
