It's not so likely to hear "prenditi cura di te" or "abbi cura di te" in our informal everyday language. The English "take care" seems to be more frequent and used in in a greater variety of contests and situations instead.
Possible alternatives are "stammi bene" or the less frequent "oh, mi raccomando...", which implies that you're about to say "abbi cura di te", although you don't say it.
