The expression "rip my eyes out" and its variations such as the phrases you use above, are actually idioms used quite often in informal conversation. They mean that something you saw or are doing has, in some way, become so painful that you would rather not see it. Since your eyes are transmitting the vision of whatever it is to your head, you want to rip your own eyes out of your head to stop seeing the offensive item. It just means that something is a horrible sight or experience. This book is so boring that I want to rip my eyes out. That man's clothing is so offensive that it makes you want to rip your eyes out. Movies that make you want to rip your eyes out. Movies that are really bad, do not go. As for the nude woman example, she must not have been at all physically appealing to the speaker.

I am not familiar with this usage, as in your examples. To me it has the meaning to physically attack someone fiercely, suggesting with claws to "rip their eyes out". For that reason, it is associated in my mind with a female, with sharp fingernails, attacking someone. Maybe it's just my guilty conscience.

Yes, it's a figurative meaning. You can tell this by the phrase "makes you want to..." You'll often find overly-dramatic expressions like this used for emphasis. The main thing is not to assume it always has a literal meaning.
