actually I think it is the same but according to different situations and the regions we use they differently. 不肯will not be used in formal (very very very formal) tongue but it will sound more native when you use it in china.（ORAL only)

不肯 = unwillingness due to external circumstances etc 不愿意 = unwillingness due to your heart
