1. forgot or have forgotten are correct. Had forgotten is wrong. The forgetting is relevant to the present situation. There is .no relevant distinction between the times of realising and forgetting. The second sentence has no clear meaning and needs to be rewritten.
2. Had forgotten is correct as it clarifies that the forgetting happened before the failures to evaluate. Forgot would also be ok because the time sequence is clear.

We should wait for natives' answers, but i suppose there is a mistake in (1). You can say "i'm sorry i'VE just realized that i forgot to bring the tape recorder"
