This refers to something or someone that happened in the past and is appearing again. This usually means something negative.
"Larry met a ghost from the past. It was that girlfriend he broke up with a year ago."

Just to clarify, Larry's ex-girlfriend is not dead. :) She simply turned up, like a ghost. With the same effect, I suppose!
