'I would be in better condition now' or 'I would be in a better place now' You can't pluralise 'conditions' here, and we don't usually put the article 'a' in front of 'condition'. The use of 'If I had worked / I would be' is correct

I would strongly suggest "If I had worked harder, I would be better off now." Even if you fix the last part (changing in a better conditions > in better condition) it sounds awkward.

Excellent...grammatically, it is good . (An unreal past condition that was not met with an unreal present consequence) Only thing I would change is your choice your of words to If I had worked harder, things would be easier now/ life would be easier now/ I would be doing better now.

No, it is grammatically incorrect. Its correct form is, If I had worked harder, I would have been in better condition. This is because if you use past participle at first in conditional sentences, then you must close it up with would + present participle.

almost correct :) “If i had worked harder, i would be in better condition now” In this sentence, the time is past in the 'if' clause, and present in the main clause. They refer to an unreal past condition and its probable result in the present.
