No difference at all, except "take care always" has a little more emphasis. But the meaning is still exactly the same. Angry and Enraged. Same meaning, different levels of intensity. Take Care Always is a little more heartfelt. Does that help? Fdmaxey is on the right track with the close relative/friend analogy. That's the sentiment.

"Take care always" is usually said to a person with whom you are close - a close relative or loved one, somebody that you care for. "Take care" would be said more to an acquaintance or colleague. You might be friendly with them, but not very close - not like a relative.
