Hi, it means to have a misunderstanding or get off to a bad start. Here's an example: let's say you and I have never met each other. You are coming to my office to interview for a job. We both try to get on a crowded elevator at the same time, but there is only room for you. You later meet me for the first time and I might say, "Sorry we got off on the wrong foot, my name is... It's good to meet you".
