I have an advanced student who, after an initial conversation-based lesson, wants to try something more structured. His grammar is quite good, but wants to work on vocabulary as well as general language skills. This student is an adult; his English is good enough to express his ideas freely, but sometimes has trouble coming up with the right word.
What types of structured classes and exercises do you enjoy? For vocabulary or otherwise.

The way I handle this is to have specific topics based around the students' area of interest/occupation as this is where it tosses up the vocabulary that is missing.
For example, my Business English students are from diverse backgrounds and I handle it by discussing what is topical in their industry and preparing a bespoke lesson to meet those needs.
Business idioms, subtle nuances in word choice etc are very popular.
