The last two images are forms. Forms are a way to ask for and collect information. They have questions and topics which have a blank space to be answered in. Many forms may ask for your name and contact details. Forms can be used when going to the doctor and they need information about you. They can also be used when applying for a job or leaving feedback for a service.
