What's the difference between "stress" and "stressed out"? Thank you.

STRESS is a noun, or possibly a verb, with many applications. STRESSED OUT is basically an adjective, and probably only used to describe people.

The first could apply to people or items. A steel beam under stress. The second can imply you're at your limit as a person.
